All information is provided for general educational purposes only and does not constitute medical advice. Only a licensed healthcare provider, following an appropriate clinical evaluation, can determine a diagnosis or treatment plan.

    3. PRIVACY POLICY & HIPAA NOTICE

Your use of the Site is subject to our Privacy Policy. We also maintain a Notice of Privacy Practices in compliance with HIPAA.

Important: Website forms, emails, and online submissions are not intended for transmitting Protected Health Information (PHI). Do not submit sensitive medical or diagnostic information through the Site. HIPAA-protected communications occur only through secure clinical systems after you become an established patient.

    4. MEDICAL DISCLAIMERS

    4.1 No Doctor–Patient Relationship

A doctor–patient relationship is established only after an in-person or approved telemedicine consultation and execution of required clinical documentation.

    4.2 Medical Emergencies

If you believe you are experiencing a medical emergency, do not use this Site. Call 911 or local emergency services immediately.

    4.4 Imaging & Diagnostic Limitations

Diagnostic imaging findings (MRI, CT, X-ray, ultrasound) do not always correlate directly with pain symptoms and must be interpreted within full clinical context.

    4.5 Medication Management

All medication decisions, including those involving controlled substances, are made solely by licensed providers based on clinical evaluation, medical necessity, and applicable regulations.

    4.6 Telemedicine Limitations & Consent

Telemedicine may not be appropriate for all conditions and may require in-person evaluation. Technical failures caused by third-party platforms may occur. By engaging in telemedicine services, you acknowledge and consent to the use of electronic communications for healthcare delivery and understand the inherent limitations and risks associated with remote care.

    5. COMMUNICATION CONSENT (SMS & EMAIL)

By providing your phone number or email address, you consent to receive appointment reminders, care coordination messages, and limited practice-related communications.

Consent to receive SMS or email communications is not a condition of receiving medical care or services. Opting out of marketing messages does not affect essential administrative or care-related communications where permitted by law.

    6. USER SUBMISSIONS & FEEDBACK

Any non-medical feedback or suggestions you submit may be used to improve or promote the Site and services. Medical records and PHI are excluded from this section and are governed by HIPAA and applicable healthcare laws.

    8. INDEMNIFICATION

You agree to indemnify and hold harmless The Spine & Rehab Group from claims arising out of your misuse of the Site or violation of these Terms. This provision applies solely to website use and does not limit patient rights related to licensed clinical care or medical malpractice claims where prohibited by law.

    9. COMMERCIAL REUSE OF SITE CONTENT

You may not copy, reproduce, distribute, or commercially exploit Site content without prior written authorization. This restriction does not prohibit fair use, indexing by search engines, or non-commercial educational referencing as permitted by law.

    10. MODIFICATIONS TO TERMS & SERVICES

We reserve the right to modify these Terms at any time. Updated Terms will be posted with a revised “Last Updated” date. Continued use of the Site constitutes acceptance of the revised Terms.
